RUMFORD — The River Valley Healthy Communities Coalition is now accepting nominations for the Rumford Heart Walk’s 2014 Lifestyle Change Award.
It will be presented at the walk on Sunday, Sept. 14, at Hosmer Field in Rumford, Sandra Witas, the coalition’s Heart Walk team captain, said Wednesday in an email.
“Do you know someone who has changed his or her lifestyle to improve heart health?” she asked of nominators.
The American Heart Association and the Rumford Heart Walk want to recognize people who have made positive changes to improve their quality of life and health. These improvements are making a difference in their physical health, changing the direction of their life.
“Maybe that someone is you!” Witas said.
Friends, co-workers or relatives can nominate positive role models for the Lifestyle Change Award. People may even nominate themselves.

The nomination form is simple to fill out, she said. It asks for the phone number and email address of the nominator and the nominee, along with details about the ways the nominee has changed his or her lifestyle.
The deadline for submitting the form is Friday, Sept. 5.

The Heart Walk is sponsored nationally by Subway, regionally by Hannaford Supermarkets and locally by River Valley Healthy Communities Coalition, Rumford Hospital and Oxford Federal Credit Union.
